Qualifying score on the WorkKeys assessment and a copy of high school diploma/GED. If you are enrolled through COTC a copy of your student ID will be required, and a signed release from advisor.

Note: You must wear royal blue scrubs during all classes and clinicals. Requirements for Class and prior to Clinic, bring a copy of the following when you begin class:

Driver’s license or state issued ID,BCI background check code #17338 processed within the last  6 months (allow a minimum of 30 or more days to receive results from the state). The Ohio Department of Health will also ask to see your original Social Security card at the time of your state certification test. 

If a student has never received a 2 step TB skin test, or if it has been longer than 12 months since their last test, a new 2 step Mantoux skin test will be required or if a student is currently working as a healthcare provider and has participated in annual TB testing they will need the following. 1. The most recent annual result which must be within 12 months of the last day of class. 2. The student must also have a copy of the previous annual TB test result. All students must be able to lift 50 lbs.; a physician release may be required in some cases such as pregnancy.
